We are excited to announce an opening for a Care Manager at our South Lakeland office! In this role, you are responsible for establishing a systematic process and criteria to identify patients who may benefit from care management. Responsibilities include but are not limited to: Implementing a comprehensive, ongoing, and coordinated approach to achieving desired outcomes for a population of patients based on disease group: i.e., CHF, COPD, DM, Schizophrenia, Cancer, etc.
Key Responsibilities:
- Perform telephonic Case Management, which includes patient clinical assessment, education regarding disease and early symptom identification.
- Participates in the identification, collection, and analysis of data for potential enrollment of members to the Disease Management program.
- Applies critical thinking skills and problem-solving techniques to the disease management process.
- Accurately conducts a thorough and objective holistic assessment of the patient’s current status, including psychosocial, physical, financial, and educational needs, prognosis, and past and present treatment courses and services.
- Partners with physicians for effective clinical management and utilization of resources for the patient population.
- Provides clinical direction and coordination to the interdisciplinary team to formulate and implement a plan of care.
- Maintains current knowledge of disease processes, available resources, and treatment options appropriate to patient population.
- Consistently documents all activities according to clinical guidelines, including automated systems.
- Continuously demonstrates a positive attitude and understanding of case/disease management, supports team building, and is motivated to fulfill department objectives.
- Consistently demonstrates excellent communication and interpersonal skills with supervisor, case manager, physicians, clinical coordinator, and corporate personnel.
- Identifies opportunities and readily participates in performance improvement activities.
- Acts as a liaison for members as needed.
- Ability to educate members as well as clinic personnel on the DM Program.
- Present to Management Disease Management Program and Update in Impact to current population.
- Be a key contributor to NCQA and URAC accreditation for Disease Management.
Requirements
- High school diploma or General Education Degree (GED).
- 1 year of experience as a Medical Assistant.
- 1 year of related experience in outpatient clinic, hospital setting, insurance, medical records, billing, or related area preferred.
- Six to twelve months of data entry experience required.
- Intermediate-level MS Excel skills are required.
- Working knowledge of medical terminology preferred.
- Experience with hospital portal systems preferred.
- Effective interpersonal skills are required.
- Good organizational and time management skills.
- Effective Communication Skills (Written and Oral)
- Should possess good Phone Skills.
- Must be Detailed Oriented.
- Bilingual (English/Spanish)
